Took off from Little Staughton at 10.42. Was on a fighter affiliation exercise. Reported to have lost control whilst making a diving turn at 8,000 feet and dived into the ground at 11.32. All the crew were killed.

Flight Sergeant 417981, Royal Australian Air Force. Died in air accident Saturday 23 September 1944. Age 24. Farmer before enlisting. Son of William and Una Myrtle Kerr Mitchell; husband of Aileen Elizabeth Mitchell, of Kyancutta, South Australia. Buried: BROOKWOOD MILITARY CEMETERY, Surrey. Ref. 4. O. 11. Reference to be found in the Australian War Memorials AWM148 Roll of Honour cards, 1939-1945 War, Air Force. Commemorated on the Kyancutta Roll of Honour.

Lancaster PB512. 582 sqn. Crashed nr. Chipping Ongar, Essex 23.9.44 whilst on a Pathfinder training mission. Took off from Little Stoughton on fighter affiliation excercise, lost control while in diving turn at 8000ft. Dived into the ground at 11.32am 4 miles East of North Weald airfield, all on board killed.
